Hammersmith & Fulham Council’s Public Health Team are reviewing the Health Visiting Service to ensure it meets the needs of local babies, children, and their parents and carers.
We want to understand more about Hammersmith & Fulham families’ experiences of NHS health visiting so that we can include families in creating the best possible service.
Your voice as a parent or carer is very important. This short survey should only take about 10-15 minutes of your time and you could win a £50 voucher in our prize draw to use nationally and locally in shops. If you want to be entered in this prize draw you will need to add your email address at the end of the survey.
All responses to the survey questions will be treated as strictly anonymous and confidential and will only be used for the purposes of this consultation. Further information is available in our privacy policy.
